This agreement between ________________________________ (Employer) and Johanson Group, executive and management personnel placement service provider, is effective this _____ day of ______________, 20___ and will last for 90 days from the effective date and/or successful candidate placement within 180 days of the effective date.

Employer agrees to review resumés of potential executive and professional candidates in strict confidence and will inform Johanson Group of its desire to contact potential candidate(s) prior to making such contact.

Employer agrees to compensate Johanson Group for placement fees equal to twenty percent (20%) of successful candidate’s first year base salary. This fee is due upon the successful candidate’s first day of employment and is payable to Johanson Group by the employer.

Nothing in this agreement offers a guarantee that the successful candidate selected by Employer will stay in the Employer's hired position for any given time period. This placement fee is recognized by the Employer and Johanson Group as a fee for providing potential candidates for Employer's position opening(s) and providing candidate feedback summary.

Employer agrees to abide by the above provisions and desires to obtain resumes from Johanson Group for the following candidate number(s): ___________________________

Upon receipt of this completed and signed form by Employer, Johanson Group will fax or e-mail requested candidate resumé(s) to Employer within one business day.
